Disconnected, duplicate, wrong person, not eligible, not interested, no consent evidence, and unreachable are different failures. Collapsing them into one code makes the vendor impossible to manage and the agent easy to blame.

Feedback must return upstream

Content Marketing Institute’s 2025 B2B survey found 47% of respondents struggled to measure content results and 43% struggled to align sales and marketing. The population is not insurance lead operations, but the organizational pattern is familiar: downstream learning often fails to reach upstream decisions. [1]

I want a small, defined disposition set; evidence where appropriate; consistent coaching; and source-level outcome reviews. When feedback changes buying, scoring, and routing, the graveyard becomes a laboratory.
